This is so we can understand whether you are eligible for the service, and so we know what to call you.
What you would like support with
This could be more than one thing; for example, weight management and smoking. We ask at this stage so we know how to progress your application.
Gender
Gender can be a factor in BMI calculations for child weight management, but we also collect it for monitoring purposes.
Employment type
This is for monitoring purposes. We support anyone who qualifies for our help, but we also want to make sure we are supporting the people who need us most.
Your email address and phone number
Email will be the main way we contact you about the service, with phone as a backup.
Ethnicity
Ethnic origin can be a factor in BMI calculations for weight management, but we also collect it for monitoring purposes. As with employment type, we want to be sure we are supporting those who need us most.
Sexual orientation, marital status, and disability or mental health issues
We ask about disability and mental health issues so we can adapt our services to your needs.
Data about sexual orientation and marital status is just for monitoring purposes — to make sure we are reaching everyone who might need our help.
Height and weight
This allows us to calculate your body mass index (BMI), so we can tell whether you are eligible for the service.
Drinking habits
If you have requested help with alcohol, we ask a series of questions about your drinking — to tell us what level of support you might need.
